Wegovy, Ozempic, Zepbound and Mounjaro are injected under the skin (subcutaneously) of the stomach area, the thigh, or the upper arm, and all four labels say to rotate your injection site with each dose. On the stomach, the Instructions for Use say to stay at least 2 inches (5 cm) from your belly button. For the upper arm, the Zepbound and Mounjaro labels, and the Wegovy and Ozempic syringe instructions, say someone else should give the injection in the back of the arm. The labels report similar drug levels whichever of the three areas you use.

## Key takeaways

- All four labels list the same three areas: stomach (abdomen), thigh, and upper arm, injected under the skin, never into a muscle or vein.
- On the stomach, the Instructions for Use say to keep at least 2 inches (5 cm) away from the belly button.
- Zepbound and Mounjaro say the back of the upper arm should be injected by another person. The Wegovy and Ozempic prefilled syringe instructions say the same; their pen instructions list the upper arm without that note.
- Rotate with each dose. You can use the same body area each week, but not the exact same spot.
- The labels report similar exposure in the abdomen, thigh and upper arm, so the site you choose does not change how much medicine reaches your body.
- Injection site reactions such as redness, itching or bruising were reported in about 1% to 8% of people in trials, depending on the drug.

## Where can you inject a GLP-1?

You can inject in three areas: the stomach, the front of the thigh, and the upper arm. Each label adds a few details, and they differ slightly by brand and by device, so read the Instructions for Use that came with your pen, syringe or vial.

| Site | What the Instructions for Use say | Who can inject there |
|---|---|---|
| Stomach (abdomen) | Stomach area (the Wegovy pen says lower stomach), at least 2 inches (5 cm) from the belly button | You or another person |
| Thigh | Front of the upper leg | You or another person |
| Upper arm, Zepbound and Mounjaro | Back of the upper arm | Another person should give the injection |
| Upper arm, Wegovy and Ozempic pens | Upper arm | Listed without a note about who injects |
| Upper arm, Wegovy and Ozempic prefilled syringes | Back of the upper arm | "Only with help from someone else" |

The reason for the upper-arm note is practical: the back of your own arm is hard to see and reach, and pinching or holding the skin there with one hand is awkward. If you live alone, the stomach and thigh are the sites the labels let you use yourself with any of the four medicines.

All four labels are clear that these medicines go under the skin only. They should not be injected into a muscle or a vein.

## Why should you rotate GLP-1 injection sites?

Rotating gives each patch of skin time to recover. The Zepbound and Mounjaro vial instructions explain the main reason: changing your injection site within the area you choose for each dose reduces your risk of lipodystrophy (pits in the skin or thickened skin) and localized cutaneous amyloidosis (skin with lumps). These changes can build up when the same spot is used again and again.

What rotation means on the labels:

- **Change the site every week.** The Wegovy, Ozempic, Zepbound and Mounjaro Medication Guides all say not to use the same site for each injection.
- **The same area is fine.** You may inject in the same body area each week, as long as it is not the exact same spot. The Ozempic label says to use a different injection site each week when using the same body region.
- **Keep it separate from other shots.** If you also use insulin, the Ozempic and Mounjaro labels say both can go in the same body area, but not right next to each other. The Zepbound vial instructions say not to inject in the same site used for other medicines.

A simple habit is to picture each area in quarters and move clockwise, or to alternate left and right sides each week. Keeping a record of where you injected last makes rotating automatic.

## Does injection site affect how well a GLP-1 works?

No, according to the labels. The Wegovy and Ozempic labels say similar exposure was achieved with injection in the abdomen, thigh or upper arm, and the Zepbound and Mounjaro labels say the same for tirzepatide. Exposure means how much medicine reaches your bloodstream over time.

The Wegovy label also says the injection site can be changed without any change to the dose. So there is no "best" site for weight loss. Choose the one that is comfortable and easy for you, and rotate.

## What skin reactions are normal after a GLP-1 shot?

Mild, short-lived reactions at the injection site are possible and usually not serious. The labels describe them as redness, itching, bruising, pain, rash, inflammation, irritation or a hardened spot. How often they happened in the trials varied by drug:

| Medicine | Injection site reactions in trials | Placebo |
|---|---|---|
| Wegovy (weight trials, adults) | 1.4% | 1% |
| Ozempic (placebo-controlled trials) | 0.2% | Not reported |
| Mounjaro (placebo-controlled trials, adults) | 3.2% | 0.4% |
| Zepbound (weight trials) | 6% to 8%, depending on dose | 2% |

The Zepbound and Mounjaro labels add that reactions were more frequent in people who developed antibodies to tirzepatide.

A small drop of blood after the needle comes out can happen. The Instructions for Use say to press the spot lightly with a cotton ball or gauze, and the Ozempic pen and Wegovy FlexTouch instructions say not to rub the area.

## When should you call about an injection site reaction?

Call your clinician if a reaction is getting worse, spreading, or not fading after a few days. The same goes for any skin that turns pitted, thick or lumpy where you usually inject, since that is what rotation is meant to prevent.

> If you notice swelling of your face, lips, tongue or throat, a severe rash or itching, problems breathing or swallowing, a very fast heartbeat, or you feel faint after a dose, get medical help right away. The labels list these as signs of a serious allergic reaction.

## How do you prepare the skin before a GLP-1 injection?

Clean, healthy skin is the goal. The Instructions for Use for all four medicines share a few basics:

- **Wash your hands** before you start.
- **Clean the site** with an alcohol swab, or soap and water for the Wegovy pen, and let it dry before injecting. Do not touch it again after cleaning.
- **Skip damaged skin.** The Wegovy pen instructions say not to inject where the skin is tender, bruised, red or hard, and to avoid scars and stretch marks. The Zepbound and Mounjaro vial instructions say not to inject where the skin is tender, bruised, scaly or hard, into scars or damaged skin, or where the skin has pits, is thickened, or has lumps.
- **Check the medicine first.** It should be clear. Wegovy and Ozempic should be colorless; Zepbound and Mounjaro can be colorless to slightly yellow. Do not use it if it is cloudy, discolored or has particles.
- **Pinch only if you were told to.** The Zepbound and Mounjaro vial instructions say your healthcare provider should tell you whether to pinch the skin. The Wegovy prefilled syringe instructions describe pinching a fold of skin.

On temperature: the Zepbound and Mounjaro pen instructions say it is not necessary to warm the pen to room temperature before injecting. The Wegovy and Ozempic instructions do not ask you to warm the pen either. Each label has its own storage rules, including how long a pen can stay out of the fridge, so check yours.

If injecting into your lower stomach is difficult, the Wegovy pen instructions suggest switching to a firmer site, such as the upper leg or upper arm, or standing up while you inject into the stomach.

## What else do the labels say about injecting?

Training comes first. All four labels say your healthcare provider should show you how to inject before your first dose. The Wegovy, Zepbound and Mounjaro labels add that if your device changes, you should get new training and read the new Instructions for Use.

The Wegovy FlexTouch pen and the Zepbound and Mounjaro KwikPens are not recommended for self-injection by people with visual impairment unless their healthcare provider decides they can use them properly. The Ozempic pen and the Zepbound and Mounjaro KwikPens should never be shared with another person, even if the needle is changed.

For anything about your dose, timing, or a missed shot, follow your prescriber and the label.

## How Morning helps

Morning remembers your last injection sites and suggests the next one, so rotating takes no thought. Each shot is logged with its site and the dose plan you and your clinician set, with a reminder on shot day. If a site gets red or itchy, a side-effect check-in records it, and the doctor report brings that history to your next appointment. Ask Sunny can answer questions about side effects any time, and never gives dose advice.

## Questions

### Where is the best place to inject Wegovy or Zepbound?

There is no single best place. The labels report similar drug levels in the stomach, thigh and upper arm, so choose a site that is comfortable and easy for you, and rotate each week.

### Does injecting in the stomach work better for weight loss?

The labels do not support that. The Wegovy, Ozempic, Zepbound and Mounjaro labels say similar exposure was achieved in the abdomen, thigh and upper arm.

### Can I inject my GLP-1 in the same spot every week?

The labels say no. You can use the same body area, but choose a different spot each time. The Zepbound and Mounjaro instructions say rotating lowers the risk of pitted, thickened or lumpy skin.

### Can I inject Zepbound or Mounjaro in my own arm?

The Zepbound and Mounjaro labels say another person should inject in the back of the upper arm. On your own, the labels list the stomach and the thigh.

### Is bruising after a GLP-1 injection normal?

Bruising is one of the injection site reactions listed on the Zepbound label, and it is usually mild. Call your clinician if a reaction keeps getting worse or does not fade after a few days.
