The Wegovy pill is semaglutide, the same medicine as the Wegovy injection, taken as a tablet once a day instead of a shot once a week. In the US it is approved for adults to manage weight and to lower the risk of heart attack and stroke in people with heart disease and obesity or overweight. The label steps the dose from 1.5 mg to 25 mg daily over 90 days, and in its 64-week trial people on 25 mg lost an average of 13.6% of their body weight, compared with 2.4% on placebo. The label is strict about how it is taken: on an empty stomach in the morning, with a small amount of water, then a 30-minute wait.

## Key takeaways

- The Wegovy pill is oral semaglutide made by Novo Nordisk, taken once daily. It is approved in the US for adults only.
- The label's dose steps are 1.5 mg (days 1 to 30), 4 mg (days 31 to 60), 9 mg (days 61 to 90), then 25 mg daily from day 91.
- The label says to take it in the morning on an empty stomach with up to 4 ounces of plain water, swallowed whole, then wait at least 30 minutes before eating, drinking or taking other oral medicines.
- In the 64-week OASIS 4 trial (307 adults without diabetes), average weight change was -13.6% on 25 mg vs -2.4% on placebo, and 47% of people on the pill lost 15% or more.
- Side effects are mostly gut symptoms, similar in type and frequency to the Wegovy injection. About 7% on the pill stopped because of side effects vs about 6% on placebo.
- Rybelsus, and more recently Ozempic tablets, are oral semaglutide for type 2 diabetes, not for weight management.

## How is the Wegovy pill different from the injection?

The medicine is the same, semaglutide, but the way it gets into your body is very different. The injection goes under the skin once a week. The pill is swallowed every day, and only a small share of it is absorbed.

Semaglutide is a protein-like molecule that the stomach normally breaks down. The tablet includes an ingredient called SNAC (salcaprozate sodium) that helps it be absorbed, mostly in the stomach. Even so, the label estimates that only about 1% to 2% of each dose reaches the blood. That is why the daily pill dose (25 mg) is much higher in milligrams than the weekly shot (2.4 mg), and why the instructions for taking it matter so much.

| | Wegovy pill | Wegovy injection |
|---|---|---|
| How often | Once daily | Once weekly |
| Steps on the label | 1.5, 4, 9, then 25 mg, every 30 days | 0.25, 0.5, 1, 1.7, then 2.4 mg, every 4 weeks |
| Usual maintenance | 25 mg daily | 2.4 mg weekly (1.7 mg also listed; up to 7.2 mg for some adults) |
| Reaches maintenance | Day 91 | Week 17 |
| Food and timing | Empty stomach, plain water, 30-minute wait | Any time of day, with or without meals |
| Approved ages | Adults | Adults, and ages 12+ for weight |
| Approved uses | Weight management; heart attack and stroke risk in heart disease with obesity or overweight | Same, plus MASH, a liver condition |

The label also notes that semaglutide levels in the blood vary more from person to person on the pill than on the shot. Switching between the pill and the injection is possible under the label, but it has its own rules, and it is your prescriber's call.

## What are the Wegovy pill dose steps?

The label starts the pill at 1.5 mg once daily for 30 days and raises it every 30 days until the maintenance dose of 25 mg.

| Days | Daily tablet dose on the label |
|---|---|
| 1 to 30 | 1.5 mg (starting dose) |
| 31 to 60 | 4 mg |
| 61 to 90 | 9 mg |
| 91 onward | 25 mg (maintenance) |

The slow climb is there to reduce gut side effects. The label lets prescribers delay a step if a dose is not tolerated, and it names other options if 25 mg is not tolerated. Your prescriber decides your dose and when it changes. Our [titration schedules tool](/tools/titration-schedules) shows this ladder next to the injection's.

## How does the label say to take the Wegovy pill?

The label describes a specific routine, because food, drinks and other pills in the stomach reduce how much semaglutide is absorbed. According to the label:

1. The tablet is taken once a day, in the morning, on an empty stomach.
2. It is swallowed whole with plain water, up to 4 ounces (about half a cup). No other liquids.
3. It is not split, crushed, chewed or dissolved.
4. After taking it, the label says to wait at least 30 minutes before eating, drinking anything or taking other oral medicines.
5. Only one tablet is taken per day.

The label's pharmacology data explain why. Absorption was higher with a small amount of water than with a full glass, and higher the longer people waited before eating. If you take other morning medicines, such as thyroid medicine, ask your prescriber or pharmacist how to fit them around the pill. The label reports that one study found levothyroxine levels rose by about a third when taken with oral semaglutide.

For missed tablets, follow your prescriber and the label.

## How much weight do people lose on the Wegovy pill?

In the trial behind the approval, people on the 25 mg pill lost an average of 13.6% of their body weight at 64 weeks, compared with 2.4% on placebo. The trial, called OASIS 4 and published in the New England Journal of Medicine in 2025, enrolled 307 adults with obesity, or overweight plus at least one weight-related condition, and without type 2 diabetes. Everyone also got advice on diet and activity.

| Outcome at week 64 | Wegovy pill 25 mg (205 people) | Placebo (102 people) |
|---|---|---|
| Average weight change | -13.6% | -2.4% |
| Lost 5% or more | 76.3% | 31.3% |
| Lost 10% or more | 59.8% | 14.4% |
| Lost 15% or more | 47% | 5.5% |
| Lost 20% or more | 27.9% | 3.1% |

Average starting weight was about 234 lb (106 kg). Averages hide a wide range: some people lose much more and some much less. In the separate STEP 1 trial, the weekly Wegovy 2.4 mg injection averaged -14.9% at 68 weeks. The two were tested in different trials, so they cannot be compared directly. Our [weight timeline tool](/tools/glp1-weight-timeline) shows how trial curves change over time.

## What are the side effects of the Wegovy pill?

The side effects are the same kind as with the injection: mostly nausea, diarrhea, vomiting and constipation, most common while the dose is being stepped up. The label says the types and frequency of common side effects with the 25 mg pill were similar to those seen with the Wegovy injection, where nausea was reported by 44%, diarrhea by 30%, vomiting by 24% and constipation by 24%.

Some numbers specific to the pill from the label and the OASIS 4 paper:

| | Wegovy pill 25 mg | Placebo |
|---|---|---|
| Any gut side effect (OASIS 4 paper) | 74% | 42% |
| Stopped because of side effects (label) | About 7% | 5.9% |
| Stopped because of gut side effects (label) | 3.4% | 2% |
| Altered skin sensation, such as tingling or burning (label) | 5% | 0% |
| Gallstones (label) | 2.5% | 1% |

The pill carries the same warnings as the injection: pancreatitis, gallbladder problems, kidney problems from dehydration, serious allergic reactions, low blood sugar when used with insulin or sulfonylureas, and a boxed warning about thyroid C-cell tumors seen in rodents. It should not be used by people with a personal or family history of medullary thyroid cancer or MEN 2. The label also says not to combine it with other semaglutide products or other GLP-1 medicines.

> Contact your clinician or seek care for severe stomach pain that will not go away (sometimes spreading to your back), pain in your upper belly with fever or yellowing of your skin or eyes, vomiting that stops you keeping fluids down, signs of an allergic reaction, or a lump or swelling in your neck.

## Who might the pill suit?

Whether the pill or the injection fits you is a conversation with your clinician. Some people prefer to avoid needles or like a daily habit. Others find a once-a-week shot easier than a strict morning routine every day, or need the 7.2 mg injection dose or the MASH approval, which the pill does not have.

Questions worth bringing to your appointment:

- Does my morning routine, including other medicines, work with a 30-minute wait?
- Is the pill or the shot a better fit for my health and my goals?
- How will we judge whether it is working, and what side effects should make me call you?

## Is Rybelsus the same as the Wegovy pill?

No. Rybelsus is also oral semaglutide from Novo Nordisk, but it is approved for type 2 diabetes, with tablets of 3, 7 and 14 mg. In the US, oral semaglutide for type 2 diabetes is now also sold as Ozempic tablets (1.5, 4 and 9 mg). The Wegovy pill is the oral semaglutide approved for weight management, and its label reaches 25 mg. These tablets are not swapped mg for mg, so changes between them are for your prescriber. ## Questions

### Is the Wegovy pill as effective as the shot?

In its own trial, the 25 mg pill averaged -13.6% at 64 weeks; in a separate trial, the 2.4 mg shot averaged -14.9% at 68 weeks. They have not been compared head to head, so ask your clinician which fits you.

### Why do I have to wait 30 minutes after the Wegovy pill?

Food, drinks and other pills reduce how much semaglutide the stomach absorbs. The label says to wait at least 30 minutes after the tablet before eating, drinking or taking other oral medicines.

### Can I take the Wegovy pill with coffee?

The label says to take it with plain water only, up to 4 ounces, and to wait at least 30 minutes before drinking other beverages, which includes coffee.

### Is the Wegovy pill available outside the US?

This guide describes the US product and US label. Availability and approvals differ by country, so ask your clinician what is available where you live.

### Can I switch from the Wegovy shot to the pill?

The label describes how switching between the injection and the pill works, but it is a decision for your prescriber, who sets the timing and dose. Follow your prescriber and the label.
