The most useful exercise on a GLP-1 such as Wegovy, Ozempic (semaglutide), Zepbound or Mounjaro (tirzepatide) is strength training two to three times a week, plus regular walking. In the trial body scans, roughly a quarter to 40% of the weight people lost was lean mass, and strength training is the main way to protect muscle while you lose fat. The US Physical Activity Guidelines recommend muscle-strengthening work for all major muscle groups on at least 2 days a week and 150 to 300 minutes of moderate activity, and the 2025 joint advisory on GLP-1 nutrition aims for strength training at least 3 times a week. Start low, build slowly, and go easier on days when you have eaten little or feel queasy.
Key takeaways
- Strength training comes first. The 2025 joint advisory says GLP-1 medicines should be prescribed with a structured exercise program and that aerobic activity alone does less to preserve lean mass.
- The US Physical Activity Guidelines recommend muscle-strengthening activity for all major muscle groups on 2 or more days a week; the advisory aims for at least 3.
- For aerobic activity, the guidelines recommend 150 to 300 minutes a week at moderate intensity, such as brisk walking. Some activity is better than none.
- A beginner week can be 2 to 3 short full-body strength sessions on non-consecutive days, plus walks on most other days.
- On low-intake or nausea days, do a lighter session or a gentle walk, eat a small protein snack first if you can, and sip fluids.
- Stop and seek care for chest pain, fainting, severe shortness of breath or a racing heartbeat that does not settle.
Why does strength training matter most on a GLP-1?
Because some of the weight you lose on a GLP-1 is lean mass, and strength training is the signal that tells your body to keep muscle. In the SURMOUNT-1 body-scan substudy, about 26% of the weight lost on tirzepatide was lean mass; in the STEP 1 substudy, close to 40% of the weight lost on semaglutide 2.4 mg was lean mass. Lean mass includes water and organs, not only muscle, and our muscle loss guide walks through those studies in detail.
The 2025 joint advisory from four US nutrition and obesity societies is direct about this. It says GLP-1 medicines should be prescribed together with a structured exercise program, aims for regular strength training at least 3 times a week, and notes that aerobic activity alone has a smaller effect on preserving lean mass during rapid weight loss. It also says protein alone is likely not enough without resistance training.
Exercise also seems to add to what the medicine does. In a 2021 trial in the New England Journal of Medicine, 195 adults who had lost weight on a low-calorie diet were assigned for a year to exercise, liraglutide (an older, daily GLP-1), both, or placebo. The combination lowered body-fat percentage by 3.9 percentage points, about twice as much as exercise alone or liraglutide alone, and only the combination improved fitness, blood sugar control and insulin sensitivity. The program was mostly cycling and circuit work, so it supports exercise in general rather than one specific routine.
How much exercise do you need on a GLP-1?
The baseline is the same as for any adult: strength training on at least 2 days a week and 150 to 300 minutes a week of moderate activity. Here is how the two main sources line up.
| Source | Strength training | Aerobic activity | Extra notes |
|---|---|---|---|
| US Physical Activity Guidelines (2nd edition) | At least 2 days a week, moderate or greater intensity, all major muscle groups | 150 to 300 min a week moderate, or 75 to 150 min vigorous, spread through the week | Move more, sit less; some activity is better than none |
| Older adults (same guidelines) | Same as adults | Same as adults, or as much as your conditions allow | Add balance training; set effort relative to your own fitness |
| 2025 joint advisory on GLP-1 nutrition | At least 3 times a week | At least 150 min a week moderate | Consider an exercise physiologist or strength trainer |
"Moderate" means you can talk but not sing, according to the CDC's talk test. Brisk walking is the classic example. If 150 minutes sounds like a lot, the guidelines' own consumer advice is to do what you can: even 5 minutes of activity has real health benefits.
If you have a chronic condition, the guidelines say to be under the care of a health care provider and to ask them or a physical activity specialist which types and amounts of activity suit you.
What is a good beginner exercise week on a GLP-1?
Two or three short full-body strength sessions on non-consecutive days, with walking on most other days, is a solid start. Each strength session can take 20 to 30 minutes at home.
| Day | Activity |
|---|---|
| Mon | Full-body strength, about 20 minutes |
| Tue | Walk 15 to 20 minutes |
| Wed | Full-body strength |
| Thu | Walk 15 to 20 minutes, or rest |
| Fri | Full-body strength (or a walk, if two sessions is your start) |
| Sat | Longer, easy walk, 30 minutes or as you can |
| Sun | Rest, or a gentle stroll |
A beginner full-body session, done as a circuit:
- Warm-up: 3 to 5 minutes of marching in place or an easy walk, plus arm circles.
- Squats to a chair: sit back until you lightly touch the chair, then stand. Use your hands if you need to.
- Wall push-ups: hands on the wall at shoulder height, lower your chest toward it, push back.
- Rows with a band: loop a resistance band around a sturdy post or door anchor, pull your elbows back and squeeze your shoulder blades.
- Glute bridges: lie on your back with knees bent and lift your hips.
- Step-ups or heel raises: on a low step, holding a rail for balance.
- Core: a plank against a counter or wall, 10 to 20 seconds.
Do 8 to 12 slow, controlled reps of each move, one round to start. Stop each set when you could still do about two more good reps. If you are an older adult, add a simple balance drill, such as standing on one foot near a counter, as the guidelines recommend.
How do you progress safely?
Build slowly, one change at a time, every week or two. The Physical Activity Guidelines put it plainly: inactive people should "start low and go slow," starting with lower intensity and gradually increasing how often and how long they are active.
| Weeks | Strength | Walking |
|---|---|---|
| 1 to 2 | 2 sessions, 1 round of each move | 10 to 15 minutes on most days |
| 3 to 4 | 2 to 3 sessions, 2 rounds | 15 to 20 minutes, a bit brisker |
| 5 to 8 | 3 sessions, 2 to 3 rounds; harder versions | 20 to 30 minutes, working toward 150 minutes a week |
| After 8 | 3 sessions; add weight or a heavier band | 150 to 300 minutes a week, as you are able |
Ways to make a move harder once 12 reps feel easy:
- Squats: from a lower chair, then without touching the chair, then holding a weight.
- Push-ups: from the wall to a kitchen counter, then to a sturdy table or your knees on the floor.
- Rows: a heavier band, or a dumbbell row with one hand on a table.
- Bridges: hold at the top for a few seconds, then try one leg at a time.
A trainer, physical therapist or exercise physiologist can check your form and set a plan; the advisory suggests this kind of help.
How should you exercise on low-intake or nausea days?
Scale down rather than skip: do a shorter, gentler session or a walk, and save harder work for a better day. On a GLP-1 you may eat much less on some days, and training hard on very little food and fluid can leave you light-headed or weak.
Practical steps for those days:
- Eat a small, easy protein snack an hour or so before, such as yogurt, a shake or an egg, if you can keep it down.
- Choose seated or wall versions of the moves, one round only, or a 10-minute walk.
- Skip bending, lying flat or jumping if they make nausea worse.
- Stop if you feel dizzy, shaky or sweaty in a way that is not normal for you.
If you have type 2 diabetes and take insulin or a sulfonylurea, the Zepbound and Mounjaro labels warn that combining them with tirzepatide can raise the risk of low blood sugar, and the semaglutide labels carry a similar warning. Signs include shakiness, sweating, dizziness, confusion and a fast heartbeat. Ask your clinician how to plan exercise around your medicines.
How much water should you drink when you exercise?
Start from Morning's daily target, about 30 ml per kg of body weight kept between 2 and 3 liters, and sip extra around workouts, especially in heat. GLP-1 medicines quiet thirst, so you may not feel the cue to drink. The water calculator gives your base number.
Hydration matters more than usual on these medicines. The Wegovy and Zepbound labels warn about acute kidney injury from dehydration, mostly in people with nausea, vomiting or diarrhea. On a day you have had any of these, keep exercise light and fluids up.
If you cannot keep fluids down, feel dizzy when you stand, or are peeing much less than usual, rest and contact your clinician or seek care. Severe dehydration needs prompt treatment.
When should you stop exercising and get help?
Stop right away and seek care if you have chest pain or pressure, faint or nearly faint, are very short of breath, or have a heartbeat that races or flutters and does not settle with rest. Pain in a joint that gets sharper with each rep is also a reason to stop that move.
Other symptoms that need a clinician, whether or not you were exercising:
- Severe stomach pain that will not go away, especially if it spreads to your back, which can be a sign of pancreatitis.
- Pain in the upper right belly, fever or yellowing skin or eyes, which can be a gallbladder problem.
- Swelling of the face, lips or throat, or trouble breathing, which can be an allergic reaction.
Muscle soreness a day or two after a new workout is normal; sharp pain is not.
What if exercise has not moved the scale?
That is common, and it is not a sign the exercise is wasted. The point of strength training on a GLP-1 is to protect muscle and strength while the medicine helps you lose fat, so a better measure is how easily you climb stairs or stand from a chair. If your weight has stalled for weeks, our plateau guide covers what is normal and what to raise with your clinician.
How Morning helps
Morning's weight and progress charts sync with Apple Health, so you can see your trend alongside the activity your phone or watch records. The food photo scan counts protein and water toward your daily targets, which matters most on training days. Side-effect check-ins show how you feel around shot day, so you can plan harder sessions for your better days, and the 30-day Academy has short lessons on protein and strength.

Questions
What is the best exercise on Wegovy or Zepbound?
Strength training two to three times a week, plus walking. The 2025 joint advisory notes that aerobic activity alone does less to preserve lean mass, so add resistance work that covers your legs, push, pull and core.
How often should I strength train on a GLP-1?
The US Physical Activity Guidelines recommend at least 2 days a week for all major muscle groups, and the 2025 joint advisory aims for at least 3. Leave a rest day between full-body sessions.
Can I exercise on my shot day?
Many people can. If you feel queasy around shot day, plan a light walk and keep harder sessions for better days.
Should I exercise if I have barely eaten?
Keep it light, such as a short walk or one easy round, and have a small protein snack first if you can. If you feel dizzy or shaky, stop and rest.
Is walking enough on a GLP-1?
Walking is good for your heart and mood, but on its own it does less to protect muscle. Add strength training at least 2 days a week.
