GLP‑1 shot day: what to expect and what to eat

What the 1 to 3 days after your weekly shot often feel like, what to eat, and a simple routine.

Updated · 9 min read · By the Morning team · How we write

After a weekly shot of Wegovy or Ozempic (semaglutide), the labels say the drug reaches its highest level in your blood 1 to 3 days later; for Zepbound and Mounjaro (tirzepatide), the peak comes 8 to 72 hours after the shot. Many people notice that appetite is lowest and stomach side effects such as nausea, fullness or burping are most noticeable in that window, especially in the first weeks and after a dose increase. Small, bland, protein-forward meals, steady fluids, and the same shot day each week make that stretch easier to plan around.

Key takeaways

  • The Wegovy and Ozempic labels say semaglutide peaks 1 to 3 days after the shot. The Zepbound label puts the tirzepatide peak at a median of 24 hours, in a range of 8 to 72 hours.
  • All four labels say to take the shot once a week, at any time of day, with or without meals. Wegovy and Ozempic add "on the same day each week".
  • The labels allow changing your shot day if needed: at least 2 days (48 hours) between doses for Wegovy and Ozempic, and at least 3 days (72 hours) for Zepbound and Mounjaro. Ask your prescriber first.
  • Stomach side effects are most common while the dose is going up. The Zepbound label says most nausea, vomiting and diarrhea happened during dose escalation and decreased over time.
  • Rotate your injection site every week, keep pens in the refrigerator at 36°F to 46°F (2°C to 8°C), and never use a pen that has been frozen.
  • For a missed dose, follow your label and your prescriber.

What happens in the days after a GLP-1 shot?

Drug levels climb for a day or more after your shot, then slowly come down over the week. That is the pattern behind how many people feel: a quieter appetite and, for some, stomach side effects in the first one to three days, easing as the week goes on.

What the labels say about timing:

MedicinePeak level after the shotHalf-life on the label
Wegovy (semaglutide)1 to 3 daysabout 1 week
Ozempic (semaglutide)1 to 3 daysabout 1 week
Zepbound (tirzepatide)median 24 hours (range 8 to 72 hours)about 5 days
Mounjaro (tirzepatide)8 to 72 hoursabout 5 days

The half-life is why these medicines work as a weekly shot: the level drops slowly, so there is still medicine on board when the next dose comes.

These numbers describe the medicine, not your experience. The labels do not report side effects by day of the week, and many people feel little at all. But if you notice a pattern, such as feeling queasy the day after your shot and fine by day four, you are not imagining it. Writing it down for a few weeks shows your own rhythm.

Which side effects are most likely on shot day?

The stomach side effects at the top of every label are the ones people most often link to shot day: nausea, feeling full quickly, burping, indigestion, diarrhea, constipation and, less often, vomiting. In Zepbound's weight trials, nausea was reported by 25% to 29% of people depending on dose, versus 8% on placebo. Tiredness is also on the labels' list.

Two things tend to make the days after a shot harder:

  • The first weeks, and the weeks after a dose increase. The labels say stomach side effects happened mostly during dose escalation. The Zepbound label also notes that the medicine's effect on stomach emptying was strongest after the first dose and faded with later doses.
  • Big, rich meals. When your stomach empties more slowly, a large or greasy meal can sit heavily for longer.

Injection site reactions, such as redness, itching or a small bruise, can also show up right after the shot. They are usually mild. Our injection sites guide covers what is normal.

Get medical care right away for severe stomach pain that will not go away, with or without vomiting, sometimes felt in your back (a possible sign of pancreatitis), for vomiting or diarrhea that keeps you from holding down fluids, or for swelling of your face, lips, tongue or throat or trouble breathing.

What should you eat on shot day and the day after?

Eat smaller, simpler meals, put protein first, and keep fluids going. The labels do not set a diet, but the side effects they describe point in that direction, and these habits help many people through the first days after a shot.

GoalWhat it looks likeWhy it helps
Small portionsHalf your usual plate, or 4 to 6 mini mealsA slower stomach handles less at once
Protein firstEggs, Greek yogurt, cottage cheese, chicken, fish, tofu, a protein shakeHelps protect muscle when you eat less
Bland and low-fatRice, toast, crackers, potatoes, broth soups, bananasGreasy or very rich food often worsens nausea
Fluids through the dayWater, broth, herbal tea, diluted juice; small sips if queasyVomiting and diarrhea can cause dehydration
Slow pacePut the fork down between bites, stop at "comfortably full"Fullness arrives sooner than you expect

A simple shot-day menu might be Greek yogurt with a banana in the morning, chicken and rice soup at lunch, a few crackers with cheese in the afternoon, and baked fish with potatoes at dinner, with water or broth between meals. If even that is too much, eat what you can and come back to protein when your appetite returns.

On the day after, keep the same approach. Many people find their appetite starts coming back over days two to four, which is a good time to make up some protein and fiber. The protein calculator gives you a daily target, and our nausea guide has more on settling your stomach.

Why do fluids matter so much on shot day?

Because losing fluid is one of the real risks on the labels. All four say that nausea, vomiting and diarrhea can cause dehydration, which can lead to kidney problems, and that it is important to drink fluids to reduce that chance.

On shot day, appetite is often low, and many people get a good part of their water from food. Eating less means drinking a little more on purpose. Keep a bottle within reach and sip through the day rather than drinking a lot at once, which can make nausea worse. The water calculator gives you a daily target.

Call your clinician if you cannot keep fluids down, or if nausea, vomiting or diarrhea will not go away.

How do you pick the best day for your GLP-1 shot?

Choose a day you can keep every week, and one where a slower day or two afterward fits your life. The Wegovy and Ozempic labels say to use the medicine once weekly "on the same day each week, at any time of day, with or without meals". The Zepbound and Mounjaro labels say once weekly, at any time of day, with or without meals.

Things people often weigh:

  • Your busiest days. If the day after your shot tends to be slow, avoid shooting the night before a big presentation or a long workout.
  • Weekends. Some people like a Friday or Saturday shot so the quieter days fall on the weekend. Others prefer a weekday so weekend meals and plans are not affected.
  • Who helps you. If someone else injects in the back of your arm, pick a day they are around.
  • Habit. Tie it to something weekly you already do, so it is hard to forget.

What if you need to change the day? The labels allow it under one condition. For Wegovy and Ozempic, the time between two doses must be at least 2 days (48 hours). For Zepbound and Mounjaro, it must be at least 3 days (72 hours). Talk to your prescriber before you switch, and follow your label. If you miss a dose, follow your label and your prescriber too; the rules differ by medicine.

What should be on your shot-day checklist?

A short routine keeps shot day calm. Read the Instructions for Use that came with your pen or vial, because steps differ by device.

  1. Check the pen. Make sure it is your medicine and dose, not expired, not frozen, and the liquid looks the way the instructions describe.
  2. Wash your hands and clean the site with an alcohol swab or soap and water, then let it dry.
  3. Rotate the site. Use the stomach (at least 2 inches, or 5 cm, from the belly button), the thigh, or the upper arm. You can use the same area each week, but not the exact same spot.
  4. Inject under the skin as the instructions show, never into a muscle or vein.
  5. Dispose of it safely. Put used pens and needles in a sharps container.
  6. Log it. Write down the day, the dose your prescriber set, and the site.
  7. Plan your meals and fluids for the next two days: something small, bland and protein-rich in the fridge, and a water bottle nearby.

Storage, by label. Keep new pens in the refrigerator at 36°F to 46°F (2°C to 8°C), in the original carton to protect from light, and never freeze them. If needed, the labels allow some time out of the fridge:

DeviceTime allowed out of the fridge
Wegovy single-dose penUp to 28 days, at 46°F to 86°F (8°C to 30°C), before removing the cap
Ozempic pen, after first use56 days, at 59°F to 86°F (15°C to 30°C) or in the fridge
Zepbound and Mounjaro single-dose penUp to a total of 21 days, up to 86°F (30°C)

The Zepbound and Mounjaro instructions also say it is not necessary to warm the pen to room temperature before you inject.

How Morning helps

Morning logs each shot against the dose plan you and your clinician set and sends a shot-day reminder, and it remembers your last injection sites so rotating is easy. Side-effect check-ins show how you feel around shot day, so your own pattern becomes clear after a few weeks. The AI food photo scan counts protein, fiber and water toward your daily targets on the days you eat less. Ask Sunny can help with food and side-effect questions any time, and never gives dose advice.
