Fatigue on a GLP‑1, and what helps

How common tiredness is on semaglutide and tirzepatide, what usually drives it, what helps, and when to get checked.

Fatigue is a listed side effect of both Wegovy (semaglutide) and Zepbound (tirzepatide). In the trials behind the US labels, 11% of people on Wegovy 2.4 mg reported tiredness or weakness, compared with 5% on placebo, and 5% to 7% of people on Zepbound did, compared with 3% on placebo. So most people do not feel more tired, and for those who do, the cause is often the big drop in how much they eat and drink rather than the medicine alone. Tiredness that does not lift, or that comes with dizziness, fainting or signs of low blood sugar, is worth a call to your clinician.

Key takeaways

  • On the Wegovy label, fatigue (including weakness) was reported in 11% on 2.4 mg vs 5% on placebo. In the 7.2 mg trials it was 11% on 7.2 mg, 9% on 2.4 mg and 5% on placebo.
  • On the Zepbound label, fatigue (including weakness, lethargy and feeling unwell) was reported in 5%, 6% and 7% on the 5, 10 and 15 mg doses vs 3% on placebo.
  • These medicines lower how much you eat. Very small meals, low protein, too little fluid and poor sleep can all add to tiredness.
  • Dizziness was also more common than on placebo: 8% vs 4% on Wegovy 2.4 mg, and 4% to 5% vs 2% on Zepbound. Low blood pressure was reported in about 1.3% to 1.6%, more often in people taking blood pressure medicine.
  • If you have type 2 diabetes and take insulin or a sulfonylurea, shakiness, sweating, confusion or a racing heart may be low blood sugar. Check your glucose if you can and follow your care plan.
  • Tiredness that lasts for weeks, keeps getting worse, or comes with fainting, a racing heart or very little urine needs a clinician, not more coffee.

How common is fatigue on Wegovy and Zepbound?

Fatigue affects a minority of people: about 1 in 9 on Wegovy 2.4 mg and about 1 in 15 to 1 in 20 on Zepbound in the label trials. The table shows the rates the US labels list, next to placebo, so you can see how much is likely due to the medicine.

Medicine and doseFatigue on the medicineFatigue on placeboWhat the label counts as fatigue
Wegovy 2.4 mg injection11%5%Fatigue and asthenia (weakness)
Wegovy 7.2 mg injection11% (9% on 2.4 mg in the same trials)5%Fatigue and asthenia
Wegovy 25 mg pillSimilar to the injectionNot listed separatelyThe label says side effects were similar in type and frequency
Zepbound 5 mg5%3%Asthenia, fatigue, lethargy, malaise
Zepbound 10 mg6%3%Same
Zepbound 15 mg7%3%Same

Wegovy figures come from three trials with 2,116 adults on 2.4 mg for up to 68 weeks, plus two 72-week trials of the 7.2 mg dose. Zepbound figures come from two trials (SURMOUNT-1 and SURMOUNT-2) with 2,519 adults treated for up to 72 weeks. Both labels warn that rates from different trials cannot be compared directly, so this table does not show that one medicine is easier than the other.

Notice that placebo groups also reported tiredness. Some fatigue is just everyday life. The difference between the two columns is the part that is more likely tied to treatment.

Why do GLP-1 medicines make you tired?

The labels do not explain why fatigue happens, but several things that come with treatment can each make you tired. GLP-1 medicines reduce appetite and slow how fast your stomach empties, so many people eat far less than before, sometimes without noticing. Common contributors:

  • Eating much less. A sudden drop in food means less fuel during the day. Skipped meals, or a day of a few bites, often show up as an afternoon slump.
  • Too little protein. When meals shrink, protein is often the first thing to go. Protein helps you hold on to muscle while you lose weight, and feeling weak can follow when intake stays low. The protein calculator gives you a daily target to discuss with your clinician.
  • Not drinking enough. Less food also means less water from food, and many people feel less thirsty. Nausea, vomiting or diarrhea can dry you out further. Dehydration can cause tiredness, headache and dizziness. The water calculator gives you a daily target.
  • Poor sleep. Reflux, nausea at night or simply being hungry at odd times can break up sleep, and short sleep makes everything feel harder.
  • Dose steps. Both labels say stomach side effects are most common while the dose is being raised. Many people notice they eat least in the days after a step-up, which can leave them flat for a while.
  • Low blood pressure. On the Wegovy label, problems related to low blood pressure were reported in 1.3% vs 0.4% on placebo, and fainting in 0.8% vs 0.2%. On Zepbound, low blood pressure was reported in 1.6% vs 0.1%. Both labels say this was more frequent in people on blood pressure medicine and could be linked to stomach side effects and fluid loss.

Fatigue can also have causes that have nothing to do with your GLP-1, such as low iron, thyroid problems, poor sleep from other causes, or low mood. If it does not fit the pattern above, a clinician can check.

When is fatigue worst on a GLP-1?

Many people feel most tired in the first weeks and in the days after a dose increase, when appetite drops most and nausea is most likely. The labels do not give a timeline for fatigue itself, but they do say stomach side effects happen mostly during dose escalation and ease over time. The label schedules raise the dose every 4 weeks: Wegovy from 0.25 mg up to its maintenance dose by week 17, and Zepbound from 2.5 mg, rising in 2.5 mg steps after at least 4 weeks on each dose.

Some people also notice a pattern within each week, with lower energy a day or two after the shot. Noting how you feel each day for a few weeks shows you your own pattern, and that is far more useful than any average.

What helps with tiredness on a GLP-1?

Small, steady fuel through the day helps most people more than anything else. You will probably not eat as much as before, so what you do eat matters more. Some habits that help:

  • Do not go all day without eating. Even if you are not hungry, a small meal or snack every few hours keeps energy steadier than one late meal.
  • Protein first. Start each meal with protein, such as eggs, Greek yogurt, fish, chicken, tofu, cottage cheese or beans, so it gets eaten before you fill up. Shakes or milk can help on low-appetite days.
  • Sip fluids all day. Water, broth, milk and drinks with electrolytes count. Keep a bottle in sight. Drink more on hot days, after exercise, and if you have vomiting or diarrhea.
  • Pick foods that sit well. Greasy, fried and very rich foods stay in the stomach longest and are common nausea triggers, which can put you off eating for the rest of the day.
  • Protect your sleep. A lighter, earlier dinner and not lying down right after eating can help with reflux at night.
  • Keep moving gently. A short walk often lifts energy, and strength training helps protect muscle while you lose weight. Start where you are and build slowly.
  • Stand up slowly. If you feel lightheaded when you get up, rise in stages and have a drink of water. Tell your clinician, especially if you take blood pressure medicine.

Can a GLP-1 cause low blood sugar and tiredness?

Low blood sugar is mainly a risk for people with type 2 diabetes who also take insulin or a sulfonylurea. In the weight trials with type 2 diabetes, low blood sugar (below 54 mg/dL) was reported in 6.2% on Wegovy vs 2.5% on placebo, and 4.2% on Zepbound vs 1.3% on placebo. The Wegovy label says the risk was higher with a sulfonylurea.

Signs include shakiness, sweating, hunger, dizziness, headache, confusion, irritability, blurred vision, weakness and a fast heartbeat. If you have a glucose meter, check. Follow the plan your care team gave you, and let your prescriber know, because they may adjust your other diabetes medicines.

In people without diabetes, low blood sugar is less common. In the Zepbound trial without diabetes, a glucose below 54 mg/dL was recorded in 0.3% vs none on placebo. The Wegovy label notes that people with past bariatric surgery had more serious low blood sugar events.

When should you get fatigue checked?

Talk to your clinician if tiredness lasts more than a couple of weeks, keeps getting worse, or gets in the way of work, driving or caring for others. Bring a few weeks of notes on how you feel, what you eat and drink, and your shot days.

Get medical care right away if you faint, have a racing or pounding heart, feel confused, pass very little urine or cannot keep fluids down, or have severe stomach pain that will not go away (with or without vomiting). Repeated vomiting or diarrhea can lead to dehydration and kidney injury, which both labels warn about.

Also call your clinician if tiredness comes with yellow skin or eyes, pain in the upper right belly, or fever, which can be signs of gallbladder problems, or with low mood that does not lift. Whether to hold or change a dose is a decision for your prescriber; follow your prescriber and the label.

How Morning helps

Morning's side-effect check-ins let you log tiredness alongside nausea and other symptoms, and show whether it clusters around shot day or after a dose step. The food photo scan counts protein and water toward your daily targets, so you can see when a low-energy day followed a low-fuel day. Ask Sunny, your chat coach, can suggest easy protein and fluid ideas for small appetites, and the doctor report gathers your notes to bring to your next appointment.

Questions

Is fatigue a common side effect of semaglutide?

It is listed on the Wegovy label: 11% of adults on 2.4 mg reported fatigue or weakness, compared with 5% on placebo. Most people in the trials did not report it.

Does Zepbound make you tired?

Some people, yes. The Zepbound label lists fatigue, including weakness, lethargy and feeling unwell, in 5% to 7% of people depending on dose, compared with 3% on placebo.

Does GLP-1 fatigue go away?

For many people it eases as eating, drinking and sleep settle and the dose stops rising. The labels say stomach side effects decrease over time after escalation. If tiredness lasts for weeks, ask your clinician to look for other causes.

Can not eating enough on a GLP-1 cause fatigue?

Eating far less than your body needs, especially too little protein and fluid, is one of the most common reasons people feel drained on these medicines. Small, regular meals with protein first and steady sipping help most people.

Should I lower my dose if I feel tired?

That is a decision for your prescriber. Follow your prescriber and the label; the labels allow a prescriber to delay a step-up or choose a lower maintenance dose when side effects are hard.

Is dizziness on a GLP-1 normal?

Dizziness was more common than on placebo, at 8% vs 4% on Wegovy 2.4 mg and 4% to 5% vs 2% on Zepbound. It can come from dehydration or low blood pressure. If you faint, or feel dizzy along with confusion or a racing heart, seek care.
