GLP‑1s and PCOS: what the research shows

What the trials found, how insulin resistance fits in, and the label points on pregnancy and birth control pills that matter most with PCOS.

No GLP-1 medicine is FDA-approved to treat polycystic ovary syndrome (PCOS). Wegovy (semaglutide) and Zepbound (tirzepatide) are approved for weight management in adults who meet the label criteria, and the 2023 International PCOS Guideline says GLP-1 medicines can be considered for higher weight in adults with PCOS, alongside lifestyle changes. Small trials show they lower weight and waist size in PCOS, but the evidence on insulin resistance, cycles and fertility is limited. Two label points matter most: the Wegovy label says to stop at least 2 months before a planned pregnancy, and the Zepbound and Mounjaro labels say tirzepatide can make birth control pills less reliable for 4 weeks after starting and after each dose increase.

Key takeaways

  • PCOS affects about 10 to 13% of women of reproductive age worldwide, according to the 2023 International PCOS Guideline.
  • No GLP-1 has an FDA approval for PCOS. The guideline says liraglutide and semaglutide could be considered for higher weight in adults with PCOS, following general obesity guidelines.
  • A 2024 meta-analysis of 4 placebo-controlled trials (176 women with PCOS and obesity) found GLP-1 medicines lowered waist size by about 5 cm (2 in) and BMI by about 2.4 points, but did not significantly change HOMA-IR, a measure of insulin resistance.
  • The Wegovy and Ozempic labels say to stop semaglutide at least 2 months before a planned pregnancy because it stays in the body a long time. The Zepbound label says to stop when pregnancy is recognized.
  • The Zepbound and Mounjaro labels advise switching to a non-oral method or adding a barrier method for 4 weeks after starting tirzepatide and for 4 weeks after each dose increase.
  • The guideline asks clinicians to make sure women on GLP-1 medicines use effective contraception when pregnancy is possible, because pregnancy safety data are lacking.

Are GLP-1 medicines approved for PCOS?

No. None of the GLP-1 medicines has PCOS on its label. When a clinician prescribes Wegovy or Zepbound to someone with PCOS, it is for weight management, in adults with obesity or with overweight and at least one weight-related condition, which is what those labels cover. Ozempic and Mounjaro are approved for type 2 diabetes, which is more common in PCOS.

Medical guidance does include them. The 2023 International Evidence-based Guideline for PCOS, written by an international network of experts and women with PCOS, says anti-obesity medicines including liraglutide and semaglutide could be considered, in addition to active lifestyle changes, for managing higher weight in adults with PCOS, following general population guidelines. It also asks clinicians to talk through side effects and the likely need for long-term use, since weight often returns after stopping.

What does the research on GLP-1s and PCOS show?

The trials consistently show less weight and a smaller waist, but they are small and short, and most used liraglutide, an older daily GLP-1. Very little has been published on semaglutide in PCOS and we could not find a published randomized trial of tirzepatide in PCOS.

ReviewTrials and womenCompared withWhat it found
Austregésilo de Athayde De Hollanda Morais et al., 20244 trials, 176 women with PCOS and obesityPlaceboWaist about −5.2 cm (2 in), BMI about −2.4, triglycerides and total testosterone lower; no significant change in HOMA-IR or total cholesterol
Goldberg et al., 2024 (informed the 2023 guideline)11 trials, about 1,000 womenPlacebo, metformin and othersLiraglutide, semaglutide and orlistat looked better than placebo for weight and waist; data were too limited to pool most comparisons
Han et al., 20198 trialsMetforminGLP-1 medicines did better than metformin on insulin sensitivity, BMI and waist, but caused more nausea and headache; evidence quality moderate to low

Every one of these reviews says the same thing in the end: the results are encouraging for weight, and better trials are needed for everything else, including periods, ovulation, fertility, acne and hair growth. If someone tells you a GLP-1 will fix your cycle or your skin, that goes beyond what the studies have shown.

How does insulin resistance fit in?

Insulin resistance is common in PCOS, and the guideline says women with PCOS have a higher risk of prediabetes and type 2 diabetes regardless of their weight. It recommends checking blood sugar status at diagnosis and every 1 to 3 years after that, ideally with an oral glucose tolerance test, and again when planning pregnancy.

GLP-1 medicines help the body release insulin when blood sugar rises and slow how fast food leaves the stomach. In PCOS trials, the picture on insulin resistance is mixed: GLP-1 medicines beat metformin on insulin sensitivity in one review, but did not significantly beat placebo on HOMA-IR in another. The guideline still says metformin should be considered for the metabolic side of PCOS, including insulin resistance, in adults with a BMI of 25 or more.

Food still matters. Building meals around protein, fiber-rich vegetables, beans and whole grains supports steadier blood sugar and helps you feel full on less. Our guide on what to eat has simple ideas. The 2025–2030 Dietary Guidelines suggest 1.2 to 1.6 g of protein per kg a day; the protein calculator gives your number.

What do the labels say about pregnancy?

The weight-management labels say to stop when pregnancy is recognized, and the semaglutide labels add a stopping window before a planned pregnancy. Animal studies showed possible harm to the fetus, and there is not enough human data to know the risk. The diabetes labels leave use in pregnancy to a benefit-and-risk decision, because poorly controlled diabetes in pregnancy also carries risks.

MedicineWhat the label says
Wegovy (semaglutide, shot or pill)When used for weight or heart risk, stop when pregnancy is recognized. Stop at least 2 months before a planned pregnancy because of semaglutide's long half-life.
Ozempic (semaglutide)Stop at least 2 months before a planned pregnancy because of the long washout period.
Zepbound (tirzepatide)Weight loss offers no benefit during pregnancy and may cause harm. Stop when pregnancy is recognized.
Mounjaro (tirzepatide)Data in pregnancy are insufficient. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.

The Zepbound label does not give a fixed number of weeks to stop before trying to conceive. If you are planning a pregnancy on any GLP-1, follow your prescriber and the label on when to stop, and plan it at a visit before you start trying. That visit is also a good time for the glucose test the PCOS guideline recommends.

Irregular periods do not mean you cannot get pregnant. If pregnancy is possible and you do not want it, use reliable contraception, as the guideline advises. If you find out you are pregnant while on a GLP-1, contact your prescriber promptly. Each medicine has a pregnancy registry listed on its label.

Do GLP-1s affect birth control pills?

Tirzepatide does, according to its labels. The Zepbound and Mounjaro labels say tirzepatide may make oral hormonal contraceptives less effective because it slows stomach emptying, an effect that is largest after the first dose and fades over time.

The labels advise people on the pill to switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting tirzepatide and for 4 weeks after each dose increase. Contraceptives that are not taken by mouth, such as an IUD, implant, patch, ring or shot, should not be affected.

The Wegovy and Ozempic labels do not carry this advice. Many women with PCOS take a combined pill to manage cycles or skin, so if you take one and start tirzepatide, ask your prescriber how the label guidance applies to you before your first shot.

What should you ask your clinician?

A short list makes the visit easier:

  • Is a GLP-1 a good fit for my PCOS, and what are we hoping it changes?
  • Should I have a glucose tolerance test or other labs first?
  • Which birth control works best with this medicine?
  • If I want to get pregnant in the next year or two, when should I stop, and what happens to my PCOS care then?
  • How will we check my progress beyond weight: cycles, labs, blood pressure?

Contact your clinician or seek care if you have severe stomach pain that will not go away, pain that spreads to your back, vomiting you cannot keep fluids down through, signs of dehydration, a lump in your neck, or signs of an allergic reaction.

How Morning helps

Morning's shot log follows the dose plan you and your clinician set, with shot-day reminders, so you always know when a dose increase happened, which matters for the tirzepatide birth control guidance. Side-effect check-ins show patterns around shot day, and the food photo scan counts protein, fiber and water toward your daily targets. Lab results stay in context, and the doctor report gives you something concrete to bring to a PCOS or pre-pregnancy visit.

Questions

Can I take Ozempic or Wegovy for PCOS?

No GLP-1 is FDA-approved for PCOS. Your clinician may prescribe Wegovy or Zepbound for weight management if you meet the label criteria, which the 2023 PCOS guideline says can be considered alongside lifestyle changes.

Will a GLP-1 make my periods regular?

The trials were too small to say. They show less weight and a smaller waist, and one review found lower testosterone, but more research is needed on cycles and ovulation.

How long before trying to get pregnant should I stop Wegovy?

The Wegovy label says at least 2 months before a planned pregnancy. Follow your prescriber and the label, and plan it at a visit before you start trying.

Does Zepbound stop birth control pills from working?

The label says it may make them less effective. It advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and after each dose increase.

Is a GLP-1 better than metformin for PCOS?

One 2019 review found GLP-1 medicines did better on weight and insulin sensitivity, but the evidence was moderate to low quality. The guideline still recommends metformin for metabolic features in many adults with PCOS, and some people use both; ask your clinician.

Does a GLP-1 help with insulin resistance in PCOS?

The results are mixed. One review found better insulin sensitivity than metformin, while a placebo-controlled review found no significant change in HOMA-IR.
