A weight-loss plateau on a GLP-1 medicine is expected, not a sign that it stopped working. In the STEP 5 trial, weight loss on semaglutide 2.4 mg (Wegovy) leveled off after about week 60 and then held at about 15% below starting weight through week 104. In SURMOUNT-1, about 88 to 90% of people on tirzepatide (Zepbound, Mounjaro) who lost at least 5% had reached a plateau by week 72, with a median of 24 to 36 weeks depending on starting weight. A stall of a week or two is usually water and scale noise; a flat line for several weeks after months of loss is the real plateau, and any dose question belongs with your clinician.
Key takeaways
- In STEP 5, people on semaglutide 2.4 mg lost 15.2% of body weight by week 104, versus 2.6% on placebo, and the authors note the curve plateaued after about week 60.
- In a SURMOUNT-1 analysis, median time to plateau on tirzepatide was 24.3 weeks for people starting in the overweight range and 36.1 weeks for those with class II or III obesity.
- Higher tirzepatide doses, younger age and female sex were linked to a later plateau, by a few weeks on average.
- Early stalls are common. In SURMOUNT-1, 18% of people lost less than 5% by week 12, yet 90% of them had reached at least 5% by week 72.
- Weight swings from day to day with water, salt, bowel habits and hormones. A weekly average tells you more than any single weigh-in.
- Protein, strength training, sleep, daily movement and honest food tracking help you keep what you lost and make the most of the plateau. Dose decisions stay with your prescriber and the label.
When does weight loss usually plateau on a GLP-1?
In the main trials, the weight curve flattened somewhere between about week 24 and week 72, and that is normal. The body has a strong defense system: as you lose weight, appetite signals rise and your energy needs fall, until intake and needs meet at a new, lower weight.
| Trial | Medicine | What the curve did |
|---|---|---|
| STEP 1 (68 weeks) | Semaglutide 2.4 mg | Mean loss of 14.9% at week 68 versus 2.4% on placebo; summaries of the trial describe the curve leveling off around week 60 |
| STEP 5 (104 weeks) | Semaglutide 2.4 mg | Plateaued after about week 60 and was maintained; 15.2% mean loss at week 104 versus 2.6% on placebo |
| SURMOUNT-1 (72 weeks) | Tirzepatide 5, 10 or 15 mg | Mean loss of 15.0%, 19.5% and 20.9% at week 72 versus 3.1% on placebo |
| SURMOUNT-1 plateau analysis | Tirzepatide | Median time to plateau of about 24 to 36 weeks; about 88 to 90% had plateaued by week 72 |
In the plateau analysis, a plateau meant less than 5% change in weight over a 12-week stretch and every 12-week stretch after it. That is a loose definition, so "plateau" in the trials still allows a slow drift down. It also only included people who stuck with treatment and lost at least 5%, so it describes typical responders, not everyone.
The GLP-1 weight timeline shows how the average trial curves rise and flatten, so you can see where you sit on that shape.
Is it a plateau or just a stall?
A true plateau is a flat trend over several weeks after months of steady loss. A stall is a week or two where the scale does not move, and it usually means nothing.
Body weight moves for many reasons that have nothing to do with fat:
- Water. A salty meal, a hard workout, travel, heat or less sleep can make you hold extra water for a day or more.
- Your gut. Constipation is common on GLP-1 medicines, and food and stool still in your gut weigh something.
- Hormones. Many women retain water around their period.
- Weekly rhythm. A study of daily weigh-ins found weight tends to be higher on Sunday and Monday and lower later in the week, and the authors called this normal variation, not gain.
The fix is to look at the trend, not the day. Weigh yourself at the same time, under the same conditions, and compare weekly averages or the line on a chart. If the average has not moved for three or four weeks, you are probably at a plateau.
Why do early stalls happen in the first months?
Early stalls are common and usually short. The labels start each medicine low and raise it in steps: Wegovy reaches 2.4 mg at week 17 at the earliest, and Zepbound moves up in 2.5 mg steps every four weeks or more. During those first months, many people are still on a starter dose that is not meant to be the full effect.
People also respond at different speeds. In a SURMOUNT-1 analysis, 18% of people on tirzepatide had lost less than 5% by week 12. They were not non-responders: 90% of them reached at least 5% by week 72, with an average loss of 11.0%, compared with 22.5% for faster early responders. A slow start is a reason to keep going and keep talking with your clinician, not a verdict.
What actually helps during a GLP-1 plateau?
The habits that help most are the ones that protect muscle and keep your intake honest. They will not override biology, but they make the plateau a stable, healthy place to land and support any further loss.
Protein. Protein helps protect lean mass while you eat less. Morning's starting target is 1.4 g per kg of body weight a day, the middle of the 1.2 to 1.6 g range in the 2025–2030 Dietary Guidelines for Americans. For a 200 lb (91 kg) person, that is about 127 g a day. The protein calculator gives your number, and our guide on what to eat makes it practical.
Strength training. Some of the weight lost on a GLP-1 is lean mass. Resistance training two or more days a week, as the Physical Activity Guidelines for Americans suggest, is the best tool to hold on to muscle. Muscle will not show up as lost pounds, but it shapes how you look, feel and function. Our muscle loss guide covers the evidence.
Steps and everyday movement. In SURMOUNT-1, everyone, on medicine or placebo, was counseled toward a 500 kcal a day deficit and at least 150 minutes of physical activity a week, the same weekly amount the Physical Activity Guidelines suggest. Brisk walking counts, and daily steps are an easy way to keep activity up when appetite and energy are low.
Sleep. Sleep matters more than it seems. In a small, two-week study of 10 adults eating less, cutting sleep from 8.5 to 5.5 hours meant less of the weight lost came from fat (0.6 kg versus 1.4 kg) and people felt hungrier. It is one small study, but it fits what many people notice.
Honest tracking. As appetite comes back, portions can grow without notice. A study in adults with type 2 diabetes, which used weight data to estimate intake, found that appetite rises by about 100 kcal a day for every kilogram lost. Logging meals for a week or two, without judgment, often shows what has drifted: bigger dinners, more snacks, more drinks, or less protein.
Fast weight loss raises the risk of gallbladder problems. If you have pain in your upper right belly, fever, or yellowing of your skin or eyes, or severe stomach pain that will not go away, contact your clinician or seek care right away.
Should you change your GLP-1 dose if you hit a plateau?
That is a question for your prescriber, never one to settle on your own. The labels describe maintenance doses: Wegovy 2.4 mg weekly, with a 7.2 mg weekly dose (Wegovy HD) possible after at least four weeks on 2.4 mg, and Zepbound 5, 10 or 15 mg weekly. Whether any change makes sense depends on your health, side effects, goals and how much you have already lost. Follow your prescriber and the label.
Good things to bring to that appointment:
- Your weight trend over the past two to three months, not a single reading
- How hunger and fullness feel now compared with the first months
- Side effects and when they show up around your shot day
- Your sleep, activity and protein in a typical week
- Recent lab results, if you have them
Is a plateau the end of weight loss?
Not necessarily, but for most people it marks the shift from losing to keeping. In STEP 5, weight held steady for more than 40 weeks after the plateau while people stayed on semaglutide. Staying at a lower, stable weight is a real result: it is what the trials measured as success.
It also helps to reset what progress looks like. Waist size, blood pressure, blood sugar, how clothes fit, strength and energy can keep improving while the scale stays put.
How Morning helps
Morning charts your weight with Apple Health, so you see the trend line instead of each day's swing. The AI food photo scan counts protein, fiber and water toward your daily targets, which makes honest tracking quick. The doctor report pulls your weight, shots and side effects into one page to bring to your appointment, and Ask Sunny can talk through food and habits during a plateau (never dose advice).

Questions
How long does a GLP-1 plateau last?
In STEP 5, weight on semaglutide stayed level from about week 60 to week 104 while people kept taking it. For many people, the plateau becomes their new stable weight rather than a pause before more loss.
Is my GLP-1 not working if I stopped losing weight?
A plateau after months of loss is the expected shape of the curve, not a failure. In SURMOUNT-1, about 88 to 90% of people who lost at least 5% on tirzepatide had plateaued by week 72.
Why did I gain a few pounds overnight?
Overnight jumps are almost always water, salt, food in your gut or hormones, not fat. Compare weekly averages instead of single weigh-ins.
Should I eat less to break a plateau?
Eating much less can cost you protein and muscle. Focus first on protein, strength training, sleep and honest tracking, and talk with your clinician or a dietitian before cutting further.
Can I raise my dose to get past a plateau?
Only your prescriber can decide that. Bring your weight trend and side-effect notes to your appointment, and follow your prescriber and the label.
