Hair loss on a GLP‑1: why it happens and what helps

What the Wegovy and Zepbound labels report, why fast weight loss can trigger shedding, and the habits and signs worth knowing.

Hair loss is a listed side effect of both Wegovy (semaglutide) and Zepbound (tirzepatide). In the weight-management trials it was reported by about 3% of people on Wegovy 2.4 mg, 6% on Wegovy 7.2 mg and 4% to 5% on Zepbound, compared with about 1% on placebo. Both labels say it was associated with weight reduction. The usual pattern is diffuse shedding called telogen effluvium, which tends to start 2 to 4 months after a trigger like fast weight loss and settle over 6 to 9 months.

Key takeaways

  • On Wegovy 2.4 mg, hair loss was reported by 3.3% of people (4% of women, 0.9% of men) vs 1% on placebo. On Wegovy 7.2 mg it was 5.8% (8.4% of women, 0.2% of men).
  • On Zepbound, hair loss was reported by 4% to 5% across the 5, 10 and 15 mg doses vs 1% on placebo, in 7.1% of women and 0.5% of men.
  • Both labels say hair loss was associated with weight reduction. In the Zepbound trials, no one stopped treatment because of it.
  • Shedding after a body stress like losing 20 lb (9 kg) or more usually shows up a few months later, and hair usually returns to its normal fullness within about 6 to 9 months.
  • Normal shedding is 50 to 100 hairs a day. Telogen effluvium thins hair all over but does not cause complete baldness.
  • Patchy bald spots, eyebrow or body hair loss, or shedding with other symptoms are reasons to see a clinician.

How common is hair loss on Wegovy and Zepbound?

Hair loss affects a small minority of people on these medicines, but it is more common than on placebo, and much more common in women. The figures below come from the current US prescribing information.

Medicine and doseHair loss on medicineHair loss on placeboWomen on medicineMen on medicine
Wegovy 2.4 mg injection3.3%1%4%0.9%
Wegovy 7.2 mg injection5.8%1.0%8.4%0.2%
Zepbound 5 / 10 / 15 mg5% / 4% / 5%1%7.1%0.5%

A few details are worth knowing. The Wegovy label says that in the 7.2 mg trials, one person stopped treatment permanently because of hair loss, one paused, and five had their dose lowered; none of these happened in the 2.4 mg or placebo groups. In a pediatric trial of Wegovy 2.4 mg, hair loss was reported in 4% of teens vs none on placebo. For the Wegovy pill, the label says the common side effects in its 25 mg trial were similar to the 2.4 mg injection, without a separate hair-loss figure.

The trials were run with different people and designs, so these numbers are not a head-to-head comparison between the two medicines. What they share is the pattern: more weight lost, more hair loss reported, and mostly in women.

Why does a GLP-1 make your hair fall out?

The most likely reason is the weight loss itself, not a direct effect of the drug on your hair. Both labels say hair loss "was associated with weight reduction". Losing a lot of weight, or eating much less than usual, is a well-known trigger for a type of shedding called telogen effluvium.

Each hair on your scalp cycles between growing and resting. Normally about 15% of scalp follicles are in the resting (telogen) phase. A shock to the body, such as fast weight loss, an unusual diet, illness, surgery or major stress, can push more follicles into resting at the same time. Those hairs fall out together a few months later, which is why shedding often starts well after the weight loss began.

On a GLP-1, several things can add up: you lose weight steadily, you eat less because your appetite is lower, and on hard days with nausea you may get very little protein or iron. Any of these can contribute.

How long does GLP-1 hair loss last?

For most people, telogen effluvium is temporary. Shedding usually shows up 2 to 4 months after the trigger, and the American Academy of Dermatology says hair usually returns to its normal fullness within about 6 to 9 months once the body adjusts. If the trigger keeps going, shedding can last longer.

StageWhat you may noticeTypical timing
TriggerFast weight loss, eating much lessOften the first months of treatment or dose increases
Shedding startsMore hair in the shower, on the brush or pillowAbout 2 to 4 months later
Peak and taperHair feels thinner all over, then shedding slowsOver the following months
RecoveryShort new hairs along the hairline and partMost people back to normal within 6 to 9 months

Telogen effluvium thins hair all over the scalp. It does not cause complete baldness, and new hair keeps growing underneath. Those short new hairs are a good sign. If shedding goes on for many months, or your part keeps widening, talk to your clinician, because a second cause can sit underneath.

Does protein help with hair loss on a GLP-1?

Eating enough protein and a varied diet helps your body, including your hair, but no food or supplement has been shown to prevent GLP-1 hair loss. The American Academy of Dermatology notes that not getting enough protein, iron, zinc or biotin can cause noticeable hair loss, and that hair can regrow once the body gets enough.

Practical steps that fit a smaller appetite:

  • Put protein first. Start each meal with the protein so you get it in before you feel full. The 2025–2030 Dietary Guidelines suggest 1.2 to 1.6 g per kg of body weight a day, and Morning's default target is 1.4 g. The protein calculator gives your number.
  • Do not skip meals on low-appetite days. Small portions of eggs, Greek yogurt, cottage cheese, tofu, fish, chicken or beans add up. Our guide on what to eat has easy ideas.
  • Eat iron-rich foods. Lean meat, beans, lentils and leafy greens help. If you suspect low iron, ask for a blood test rather than starting iron on your own.
  • Be careful with high-dose supplements. The AAD warns that large amounts of vitamin A or selenium can themselves cause hair loss. Biotin can also interfere with some lab tests, so tell your clinician if you take it.
  • Be gentle with your hair. Avoid tight hairstyles, harsh treatments and vigorous brushing while it is shedding.

Protein also protects muscle while you lose weight. Our muscle loss guide explains why the two go together.

Does losing weight more slowly reduce hair loss?

Probably, but it has not been tested directly with GLP-1s. Hair loss in the trials tracked with weight reduction, it was more common on the higher Wegovy dose, and fast or large weight loss is a known trigger for shedding. That does not mean you should change your dose. Your dose plan is for you and your prescriber to set, following the label. If shedding worries you, bring it to your next visit and talk about your pace of loss and how much you are eating.

When should you see a clinician about hair loss?

See a clinician or dermatologist if your hair loss does not look like even, all-over shedding, or if it comes with other symptoms. Telogen effluvium is common, but it is not the only cause of hair loss, and some others need treatment.

Book a visit if you notice:

  • Patchy loss: round or oval bald spots, which can be a sign of alopecia areata, an immune condition.
  • Loss of eyebrows, eyelashes or body hair.
  • Scalp symptoms: itching, burning, pain, redness, scaling or sores.
  • Other symptoms: unusual tiredness, feeling cold, heavy periods, a fast heartbeat or weakness, which can point to thyroid problems or low iron.
  • Shedding that keeps going past 6 to 9 months, or a part or crown that keeps widening.
  • A lump or swelling in your neck, trouble swallowing or a hoarse voice. These are thyroid warning signs in the GLP-1 labels and need prompt care.

Your clinician may check iron, thyroid, vitamin B12 or folate levels, which DermNet recommends correcting when they are off. Bringing a short history helps: when you started, when your dose changed, how fast you lost weight and when the shedding began.

How Morning helps

Morning's food photo scan counts protein toward your daily target, so on low-appetite days you can see whether you are getting enough. Side-effect check-ins let you log shedding alongside nausea or tiredness, and your weight chart shows your pace of loss in one place. The doctor report gathers your weight, shot log and lab results so you can walk into an appointment with the timeline ready. Ask Sunny, your chat coach, can suggest easy high-protein meals.

Questions

Is hair loss on a GLP-1 permanent?

Usually not. Both labels link hair loss to weight reduction, and the typical pattern, telogen effluvium, usually settles within about 6 to 9 months. If it lasts longer or looks patchy, see a clinician.

Is hair loss more common on Zepbound or Wegovy?

The labels report 4% to 5% on Zepbound, 3% on Wegovy 2.4 mg and about 6% on Wegovy 7.2 mg, vs about 1% on placebo. The trials were different, so these are not a direct comparison.

Why is hair loss more common in women on GLP-1s?

Both labels show it in women far more than men, for example 7.1% vs 0.5% on Zepbound. The labels do not explain why, and the reason has not been established.

Should I stop my medicine if my hair is falling out?

Do not stop or change your dose on your own; follow your prescriber and the label. In the Zepbound trials no one stopped treatment because of hair loss. Talk to your clinician about what you are seeing.

Do biotin or hair supplements help?

There is no evidence that a supplement prevents GLP-1 hair loss unless you are actually low in a nutrient. High doses of some vitamins can cause problems, and biotin can affect lab results, so check with your clinician first.

When will my hair grow back?

New short hairs often appear as the shedding slows. The American Academy of Dermatology says most people's hair returns to its normal fullness within about 6 to 9 months.
